NASA knew over a decade ago that insulation could damage shuttle tiles
A 1989 NASA study of tile damage to the shuttle Atlantis concluded that insulation breaking away from forward portions of the vehicle at speeds 2.5 times the speed of sound resulted in debris damaging the tiles which protect the shuttle during reentry. Inspection revealed over 700 impacts with an entire tile knocked off from beneath the crew compartment.
